Job Responsibilities for the Hospice & Home Health Social Worker:

• Deliver supportive counseling and crisis intervention, helping patients and families navigate emotional and psychosocial challenges.
• Conduct comprehensive psycho-social assessments, assist with Advance Directives, and coordinate guardianship evaluations.
• Advocate for patient rights and self-determination while collaborating with interdisciplinary teams.
• Supervise LSWs, LPCs, and students as delegated by management and in line with Ohio Counselor/Social Work Board guidelines.

Minimum Requirements for the MSW Position:

• Master’s Degree in Social Work or Counseling (required).
• Experience in Hospice or Home Health settings (required).
• Valid driver’s license (required).
