SUMMARY:
Responsible for the implementation of standards of care for medical social work services.
SOME RESPONSIBILITIES INCLUDE:
-Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.
-Carries out social evaluations and plans intervention based on evaluation findings.
-Maintains clinical records on all patients referred to social work.
-Provides information to patients or families/caregivers and community agencies.
-Serves as liaison between patients or families/caregivers and community agencies.
-Maintains collaborative relationships with the hospital personnel to support patient care.
-Maintains and develops contracts with public and private agencies as resources for patient and hospital personnel.
-Participates in the development of the total plan of care and case conferences as required.
-Assists physician and other team members in understanding significant social and emotional factors related to health problems.
-Participates in discharge planning.
-Utilizes measures to promote and maintain patient, visitor and personnel safety.
-Maintains patient, employee, physician and organization confidentiality; respects the rights, privacy and property of others.
-Demonstrates knowledge of disaster/emergency procedures and responds appropriately.
-Assumes responsibility and accountability for individual knowledge, skills, performance and behavior in accordance with hospital, division and department standards of care and policies and procedures.
-Supports the mission, values and vision of the organization.
EDUCATION/REQUIREMENTS:
Graduate of an accredited college/university with a degree in social work (MSW required).
Current licensure, LSW or LISW, in the state of Ohio.
Minimum of one year’s experience in health care, preferred home health.
Maintain a current CPR certification.
Must have a valid Ohio Driver License.
ADDITIONAL INFO:
The department has a great team and the director is well respected.
This position is in HOME HEALTH and requires the person to have the MSW due to this is a hospital setting and Joint Commission requires it.
The hours are Monday-Friday(8am-4:30pm) and the person takes one OB call shift a month.
The position will offer a range between $51K-$75K ( depending on experience) and a full benefit package.
