Job Description
Woodland Springs is seeking a dynamic individual for the Care Coordinator position at its new behavioral healthcare hospital in Conroe, TX. We treat adults with mental health and addiction disorders as well as mental health issues arising from geriatric conditions. This state-of-the-art hospital will provide a continuum of Inpatient, Partial Hospitalization, Intensive Outpatient and after-care programs.

The responsibility of the Care Coordinator is to perform the evaluation and admission process in conjunction with the on-call physician.
Job Requirements:
Education: Master’s degree in social work or counseling required, RN licensure may be used in place of master’s degree or per state regulations.
Experience: Prior experience in a psychiatric healthcare facility working with adult and geriatric patients preferred. Must have strong clinical assessment skills.
License: Current unencumbered clinical social work or counseling license. RN license may be accepted. Valid driver’s license and a favorable Motor Vehicle Report (MVR), along with proof of insurance when using own auto.
Additional Requirements: CPR and CPI certified within 30 days of employment. May be required to work flexible hours and overtime, ability to meet typing keyboard expectations.
