Physical Therapist - Home Health (Traditional and Outpatient Home Care)

A reputable home care company is seeking an Home Care Physical Therapist for their Rehab Team. This PT will be working primarily adults patients within the home health setting. Mostly independent PT role so some experience preferred, especially within home care. Growing PT program with a cohesive team of OT's and SLP's. Competitive PPV compensation package available!

Responsibilities of the Physical Therapist:

• The Physical Therapist facilitates development and rehabilitation of patients with mental, emotional, and physical disabilities by planning and administering medically prescribed occupational therapy.
• Evaluates results of physical therapy by observing, noting, and evaluating patient's progress; recommending and implementing adjustments and modifications.
• Mobility to work in a typical clinical environment, stamina to remain standing or sitting for extended periods of time.

Requirements of the Physical Therapist:

• Full-time, Direct-hire Opportunity
• Experience preferred, especially with home health
• Day shift; 30 visits per week is full-time
• Active Ohio PT license and in good standing
