The Targeted Case Manager (TCM) provides support to patients by working with the treatment team and other recovery support staff to identify needs and facilitate connections to resources, supports, and services available in the community. The TCM will make referrals to appropriate community resources to assist patients in dealing with the barriers they face in areas such as housing stability, legal challenges, access to transportation, physical health needs, economic stability, community integration and access to social support, leisure, and recreation, etc. They will maintain close communication with all involved parties for minimal interruptions in care. Also, they will work with patients to identify their own individualized recovery goals and resource connections, ensuring patients are receiving holistic support and that all parties involved in patient care are cohesively collaborating.

Qualifications

  • Bachelor's degree in Behavioral Health Sciences.
  • At least one year of direct work experience is required.
  • Certified Targeted Case Manager (TCM) is preferred. Those without TCM must complete certification within the first six months of employment.
  • This position is subject to a background check for any convictions directly related to its duties and responsibilities. We will consider for employment all qualified applicants with arrest and conviction records and will assess the details of any criminal conviction in a manner consistent with applicable law. No background check will be conducted unless and until a conditional offer of employment has been made to an applicant. This position must also successfully pass a pre-employment drug screening.
