CleanSlate Centers was founded in 2009 in response to the growing opioid epidemic. CleanSlate Centers provides physician-led, office-based treatment for individuals suffering opioid and alcohol use disorders. We provide our patients Medication Assisted Treatment (MAT), mental health counseling, and if needed coordination with community resources. Our mission is to improve the quality of life for those battling opioid and alcohol addictions!
