Job Description

Develops and implements treatment plans with drug/ alcohol addicted patients; provides a variety of interventions including individual and group counseling, crisis intervention, and other relevant harm reduction-related services to the clients. This position will help bring the concepts of harm reduction to the clinic, the patients and the community. Develops and implements substance use treatment plans. Provide a variety of interventions including individual and group counseling, crisis intervention, and other relevant sobriety-related services to the clients. Participation in facilitating ancillary services, i.e. family support groups, family education groups, continuing care, networking with community agencies, developing and updating referral and resource information. Maintenance of individual client treatment files in accordance with agency policies and procedures, including documentation of vocational/educational activities as well as all other pertinent documentation. Link all substance use patients to mental health and medical providers.

Facilitate and prepare group sessions on various topics relating to recovery. Complete all necessary documentation in a timely manner. Refer to substance use case manager for additional support services. Refer all clients who are not ready to start treatment to appropriate harm reduction program. Recruit additional clients at Needle Exchange sites and other community events. Maintain appropriate boundaries with clients. Participate and encourage harm reduction model adaptation. Assist in development of monthly reports. Maintain confidentiality of participants. Build and maintain a contact network of agencies and local community members to expedite linkages for clients. Collaborate with other social service agencies. Participate in Quality Management plan

Certification as a CAADE, CADTP, or CCAPP. Advanced Spanish communication

skills. Ability to create and maintain boundaries with clients and understand the

limitations of a professional relationship only within the work setting. Ability to

complete required program documentation on electronic medical records. Public

speaking and leadership ability. Skill in managing multiple deadlines and demands.

Ability to follow through on assignments without intensive oversight. Ability to work

independently and as part of a group. Ability to discuss sensitive matters in a non-

judgmental manner and to respect confidentiality and reporting laws. Ability to work

effectively with people of diverse cultural and educational backgrounds. Skill in

maintaining cooperative working relationships with staff members and volunteers.

Excellent oral communication skills in order to communicate with providers, co-workers, agencies, students, patients and volunteers. Ability to manage high paced practice. Effective interpersonal skills to diffuse patients who become agitated.

Ability to work effectively as a part of a multidisciplinary team. Skill at developing drug treatment plans. Demonstrated leadership skills to initiate problem solving and program implementation. Working knowledge of diverse cultures and low income populations including the homeless. Ability to work flexible hours to meet client needs. Knowledge of HIPPA compliance to client/agency records according to established professional social standards.
