Job Description

Speech-Language Pathologist (SLP) $90/hr | Flexible Schedule | Skilled Nursing Facility

Location: Lakeport, CA.

Facility: 73-Bed Skilled Nursing Facility.

Pay: $70.00 / hour (Highly Competitive).

Caseload: 7 - 9 patients / day.

Schedule: Flexible, design your own work week (4 x 10's, 5 x 8's) - mandatory Tuesday AM rehab meeting.

Position Purpose

The SLP provides and directs all aspects of quality patient care including evaluation, treatment planning, goal setting, family education, and documentation according to professional standards and facility guidelines. The SLP also acts as a rehabilitation advocate for patients and participates in quality assurance and customer service activities.

Key Responsibilities

  • Provide speech-language pathology services in accordance with professional and regulatory standards
  • Develop individualized treatment plans and deliver effective therapy interventions
  • Supervise and guide assistants, aides, and students as appropriate.
  • Communicate patient progress with families, caregivers, and interdisciplinary team members.
  • Documentation - Complete timely and accurate documentation using Net Health/Optima and Point Click Care (PCC).
  • Participate in staff education, quality improvement, and department development initiatives

Qualifications

  • Graduate of an accredited Speech-Language Pathology program.
  • Active, unrestricted CA state license (willingness to obtain).
  • Knowledge of Medicare and other payer guidelines.
  • Strong communication and documentation skills.
  • CPR certification.
  • Commitment to patient-centered, evidence-based care.

Physical Requirements

  • Ability to move intermittently throughout the workday.
  • Ability to lift 100 lbs with assistance as required by patient needs.
  • Capable of bending, squatting, pushing, and pulling for patient care activities.
  • Meets facility health and physical standards.
