Job Description

Assesses, treats, and monitors patients, performs all necessary paperwork/reports, plans and develops new programs, maintains communication with patient, patient’s family, and physician. Presents in-services, assists in general management of the department and other diverse duties as required. E


  • Establishes, documents, and provides a plan of care with functional goals based on evaluation of the examination data and patient needs. Screens patients using written and oral tests, as well as special instruments, to diagnose the nature and extent of impairment and to record and analyze speech, language, and swallowing irregularities. Develops treatment protocol/procedures, evaluation, treatment plan and goals, patient documentation and recommendation of discharging therapy services tailored to each patient’s needs. Monitors patient progress and modifies treatment as appropriate. Provides or delegates and supervises speech language pathology patient interventions in a safe, predictable manner consistent with the established evaluation and the plan of care. Selects augmentative or alternative communication methods, including automated devices and sign language and/or lip reading and teach their use to individuals with little or no speech capability. Teaches those with little or no speech capability how to make sounds, improve their voices, or increase their language skills to communicate more effectively. Performs and documents patient re-examination, re-evaluates the data, modifies and documents the plan of care. Develops effective home program to maintain and enhance the patient’s performance in their environment. Practices therapy consistent with the code of ethics of the State and National Speech/Language and Hearing Association.

  • Implements intervention plans directly and/or in collaboration with others. Communicates relevant patient care plan information to other healthcare professionals. Adapts the environment, materials and activities according to the needs of the individual and his or her social and cultural background. Monitors the individual’s response to intervention and modifies the plan as needed. Develops appropriate home and community programming to support performance and generalize within the individual’s natural environment.

  • Collaborates with healthcare providers to coordinate medical and nursing management of patient care. Promotes team concept through effective communication regarding resident condition, resident care plan and resident satisfaction with appropriate individuals that are involved in the resident’s care. Works effectively with ancillary areas as part of the team approach to patient care. Directs patient support team members effectively and appropriately. Demonstrates initiative and flexibility in the provision of patient care. Collaborates with the Care Plan team. Completes patient and family education. Attends and participates in family conferences, patient care conferences, medical rounds and as required by areas of services to establish goals, formulate discharge summaries and follow-up plans, including home programs.

  • Maintains complete and accurate records pertinent to patient initial evaluation, progress and discharge and ensures proper documentation to be in regulatory compliance. Performs necessary documentation including evaluations, progress notes, discharge notes and billing in a timely manner according to department policy. Completes administrative responsibilities such as coordinating paperwork, scheduling case management activities, or writing lesson plans. Develops and initiates referrals and responds in a timely manner to referrals.

  • Acts as a consultant to other agencies. Promotes healthy lifestyle practices for the prevention of communication, hearing, swallowing, or other upper aero digestive disorders. Fosters public awareness of speech, language, hearing, and swallowing, and other aero digestive disorders and their treatments. Advocates at the local, state and national level for access to and funding for services to address communication, hearing, swallowing, or other upper aero digestive disorders. Develops protocols and procedures for intervention programs based on current speech-language pathology theory and practice.

  • Performs other duties as assigned.

Education and/or Experience
Masters Degree (M.A. or M.S.)

Certificates, Licensure, Registrations
Current Iowa Board of Speech Pathology & Audiology license American Speech-Language-Hearing Association (ASHA) certification. Active membership in the State and National Speech/Language and Hearing Association preferred.
