Position SummaryThe Speech Language Pathologist at OSF Home Care is responsible for assessing/evaluating patients in their home environment to determine the need for home health and speech therapy needs. They will formulate progressive care plans to restore function and prevent disability following illness or injury. Case management of occupational therapy patients, their care plans, and supervising of occupational therapist assistants.Demonstrates flexibility within their day to provide the best possible experience to all patients of any age or diagnosis. Corporate PhilosophyIt is the obligation of each employee of OSF Healthcare System to abide by and promote the Corporate Philosophy, Values, Mission, and Vision of the Sisters of the Third Order of St. Francis.Primary Job Responsibilities and Accountabilities•Evaluates patients’ physical and functional status through assessment of disorders of speech, voice, language, cognition, hearing and swallowing to prevent, identify, minimize the effects; information gathering from the medical record, and interview from patient/family/caregivers. •Analyze evaluation results and sets appropriate short term and long term goals to support/develop a care plan.•Selects, administers, and monitors determined treatment. Treatments may include: thermal tactile stimulation, compensatory swallowing strategies, diet consistency recommendations, swallowing exercises, compensatory memory strategies, swallowing positioning techniques, oral motor exercises, and augmentative device recommendations.•Recommends other disciplines, programs, and supplies needed for the patient/family/caregiver.•Case manage OT patients along with supervision of PTA’s.•Reassess patients’ response to treatment and adjusts care plan accordingly; including ongoing communication and collaboration with MD, multi-disciplinary home care team, and patient/family/caregivers.•Maintains accurate and timely documentation in electronic medical records throughout care.•Determines, instructs, and issues appropriate home programs.Basic QualificationsRequired•Requires current state license to practice as a Speech Language Pathologist.•CPR certification•Proficient with electronic documentation.•Provides own transportation with auto insurance and a valid driver’s license. Abides by all laws, rules, and recommendations to comply with OSF Safe Driving Policies while on duty.•Must be able to work independently in virtual work environment and execute own workload.Preferred•One year recent speech language pathology experience.•Home Health experience.•Experience with Microsoft Outlook, Word, Excel.•Knowledge of the SLP State Practice Act and Home Care rules and regulations.
