Social Worker - Acute - (SW - Acute)
StartDate: 09/27/2021Available Shifts: 8 D Pay Rate: $1450.00 - 1900.00
2+ years of experience. 50-mile radius rule. No more than a 3-month work history gap in the last year. MSW/CURRENT AR LMSW OR LAC (LICENSED MENTAL HEALTH PROFESSIONAL). WILL NEED TO ROTATE FOR GROUPS AND ADMITS ON THE WEEKEND. BLS-MUST BE ACQUIRED WITHIN 7 DAYS OF EMPLOYMENT. The Social Worker collaborates with hospital's case management and interdisciplinary healthcare team in an effort to meet facility resource management expectations and enhance patient outcomes. Utilizes professional social work knowledge, relationship skills and interdisciplinary collaboration to assess and intervene in psychosocial issues significant to patient/families and the healthcare team's management of illness, injury and disability. Responsibilities may include but are not limited to: auditing medical records, education of patient and staff, identifying need for psychosocial interventions, assessing discharge planning needs for patients of all ages, identifying opportunities for improvement in patient care, and providing documentation of all functions as required by regulatory agencies.
