Social Worker - Community Health - (SW - Community Health)
StartDate: 04/05/2021Available Shifts: 8 D Pay Rate: $2700.00 - 6500.00
Coordinates, negotiates, procures and manages care of patients to facilitate cost effective care and patient satisfaction. Facilitates the continuum of care, works collaboratively with interdisciplinary staff, internal and external to the organization. Responsible for UM reviews for inpatient hospital stays, carrying a complex case management case load, ownership of a disease management program, pre-certification reviews, ER reviews, and pharmacy reviews.
Required Qualifications
REQUIRED SKILLS:
*LICSW (Licensed Independent Clinical SW)
* Outpatient/Community base Case Management
*Insurance CM experience
* DC License (can apply when offered)
*Working experience in DC area
