DETROIT, MICHIGAN.MUST PASS NICOTINE DRUG SCREEN - 1 year Recent experience in Acute Care Hospital setting, MI Licensed Clinical Social Worker, with LMCSWMaster's degree in Social Work from a college or university social work program approved by the Michigan Board of Social Work and accredited by the Council on Social Work Education. Currently fully licensed as a Licensed Master's Social Worker in the State of Michigan. MUST HAVE 2+ years of acute experience. 50 mile radius rule. Will work across all hospital units and ED. EMR is Cerner. Dress code is street clothes. Parking location: Will be provided by Parking & Badges Department when get badge. The Social Worker collaborates with hospital's case management and interdisciplinary healthcare team in an effort to meet facility resource management expectations and enhance patient outcomes. Utilizes professional social work knowledge, relationship skills and interdisciplinary collaboration to assess and intervene in psychosocial issues significant to patient/families and the healthcare team's management of illness, injury and disability. Responsibilities may include but are not limited to: auditing medical records, education of patient and staff, identifying need for psychosocial interventions, assessing discharge planning needs for patients of all ages, identifying opportunities for improvement in patient care, and providing documentation of all functions as required by regulatory agencies
Required Qualifications
Michigan licensed Social Worker
MI Licensed Clinical Social Worker, with LMCSW
2 years Recent experience in Acute Care Hospital setting
Experience in with Inpatient
Monday - Friday w/ weekends twice a month
Must have Cerner & Allscripts experience
Wide range of CM knowledge
Willing to work in Detroit market during current state
