Transitions of care, assessments, participation with interdisciplinary treatment team, discharge planning, patient advocacy, patient/family meetings.

Required Qualifications
AK-License required-8 to 10 weeks on hand

Preferred Qualifications
Emergency department preferred, and/or acute in-patient. LMSW or LCSW preferred, or LBSW, ACM or CCM preferred.
