The Opportunity - Social Worker

Assesses patient, client, or family needs and eligibility for services

Assesses the psychological needs of patients and provides intervention to meet those needs utilizing personal, family and community resources
- Provides individual and group counseling to clients and possibly families

Attend meetings to stay apprised of organizational communications. Notify the Director of PopulationHealth Management or other senior leadership of staff concerns; participate in resolutions

MSW, Master’s degree in Social Work from a Council on Social Work Education (CSWE) accreditedschool
