Licensed Clinical Social Worker (LCSW)

Location: Sonoran Desert, CA

Employment Type: Full-Time

Sign-On Bonus: Up to $15,000

Summary:

As a Licensed Clinical Social Worker (LCSW), you'll play a pivotal role in facilitating care continuum, ensuring patients achieve optimal health, access to care, and utilize resources effectively. Your responsibilities include conducting complex psycho-social assessments, identifying patients at risk for readmission, and implementing interventions to promote timely throughput and safe discharge. This role adheres to national standards for case management, including transition management, care coordination, regulatory compliance, and patient education. You'll precept new staff members, participate in Quality Improvement initiatives, and collaborate in multi-disciplinary patient care conferences.

Key Responsibilities:

  • Conduct complex psycho-social transition planning assessments and interventions.
  • Assist in cases involving adoptions, abuse, and neglect, referring to appropriate agencies.
  • Coordinate care and implement transition plans while communicating with patients and families.
  • Lead multi-disciplinary patient care conferences and make referrals to other departments as needed.
  • Collaborate with physicians, office staff, and ancillary departments to ensure coordinated care.
  • Provide patient education to support post-acute needs.
  • Maintain accurate documentation in the Case Management system and patient demographic records.
  • Perform other duties as assigned.

Qualifications:

  • Required: California LCSW license.
  • Preferred: Accredited Case Manager (ACM) certification.
  • Minimum of two years of acute hospital experience preferred.
  • Strong organizational skills with excellent verbal and written communication abilities.
  • Ability to lead and coordinate activities in a fast-paced environment.
  • Proficient in critical thinking, problem-solving, and computer literacy.

Compensation and Benefits:

Base Salary: USD $81,000 to $125,548 annually.

Experience and Education:

1+ to 2 years of experience in a relevant field.

Minimum Education: Master's Degree.
