Matrix Providers is hiring a Licensed Clinical Social Worker - BHOP to join our team of talented professionals who provide health care services to our Military Service Members and their families at McConnell AFB in Wichita, KS.

  • Setting: Patient-Centered Medical Home (PCMH), Primary Care
  • Role: Internal Behavioral Health Consultant (IBHC) / Behavioral Health Care Facilitator
  • Compensation: $36.00 to $40.00 per hour
  • Schedule: Monday - Friday, 7:30 AM to 4:30 PM,
    • 40 hours per week.
    • (No Nights, No Weekends, No on-call)

As a valued member of our Behavioral Health Team, the Licensed Clinical Social Worker will:

  • Conduct brief cognitive/behavioral interventions in primary care, adhering to the AF consultation BHOP model.
  • Provide classes in a primary care-appropriate format, covering stress management, sleep enhancement, and chronic pain.
  • Contact referred patients to assess medication treatment adherence for depression, anxiety, and PTSD disorders.
  • Use validated brief symptom inventories to assess current symptom severity.
  • Assess barriers to treatment adherence and assist patients in problem-solving solutions.
  • Provide patients with education regarding their behavioral health condition and treatment regimen.

Additional Duties:

  • Maintain patient information in a patient registry with accuracy and current notes in electronic medical records for review by the managing physician.
  • Coordinate appropriate referrals to community and other Government agencies for services.
  • Independently assess the psychosocial functioning and needs of patients and their family members and formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills, and assistance needed in collaboration with the patient, family, and interdisciplinary treatment team.

LSCSW Summary:

  • Independently conduct psychosocial assessments and provide psychosocial treatment to individuals from various socio-economic, cultural, ethnic, educational, and other backgrounds.
  • Use medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
  • To achieve treatment goals, implement treatment modalities in working with individuals, families, and groups experiencing various psychiatric, medical, and social problems.
  • Independently provide counseling and psychotherapy services to individuals, groups, couples, and families. Must practice within the bounds of the license or certification.
  • Provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
  • Participate in professional peer review case conferences, research studies, or other organizations required at the MTF.

Licensed Clinical Social Worker Requirements:

  • Degree: Master's degree in Social Work (MSW)
  • Licensure: Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker.
  • Education: Graduate from a School of Social Work fully accredited by the Council on Social Work Education (CSWE)
  • Experience: One year of experience in the last three years in evaluating and treating patients who have pain, psychological factors affecting their physical condition, or difficulty coping with a medical problem. Must have experience in brief behavioral interventions.
