Job Description
Looking for a Case Management- Social Worker Coordinator. Responsible for providing social service and discharge planning functions for the hospital. Provide resources for inpatients, outpatients, and Emergency Department patients.
Must have 3+ years of previous hospital experience in all social service aspects and discharge planning.
Must have Licensure in Mississippi. BLS required.
