Job Description

NEW! LCSW NEEDED! Full-time. Days.

Salary: $62,000 annually (Base)

Description: Provide Professional Social Work services to patients and families for direct counseling.

Duties: Demonstrate communication skills, patient confidentiality, work with peers in a team situation, and understand and utilize an EMR system.

Conduct comprehensive psychosocial assessments, develop care plans, and provide counseling and crisis intervention. Coordinate services with appropriate clinic and community resources, and provide education about health/mental issues.

Requirements: Minimum of two years’ experience in social work and broad background, Master’s degree in social work, Current Indiana social work license, and ability/availability to work on call when scheduled (as needed).
