ESSENTIAL JOB DUTIES
- Assists with discharge planning primarily in Acute Care.
- Work with IDT to identify needs.
- Discuss/arranges options, services, and resources to help with areas of need.
- Assist with Health Care Power of Attorney documents and other Advanced Directives as able.
- Complete needed, required, and professional documentation and paperwork for documentation of patients' charts and other jobs tasks and requirements.
- Perform other duties as requested.
