Manager - Social Services Case Management (LCSW)

Employment Type: Full-Time, On-Site

Sign-On Bonus: $10,000

Job Summary:

As the Social Services Case Management Manager (LCSW), you will play a pivotal role in developing and overseeing social services programs. Your responsibilities will include establishing and maintaining standards, ensuring quality, cost-effective services, and fostering a customer-focused approach. Additionally, you will collaborate with community services to enhance support for our members.

This role offers an exciting opportunity to lead a team of 14 full-time medical social workers, fostering a collaborative and supportive team environment. The manager is committed to providing leadership training and support, making this an ideal opportunity for aspiring leaders in the field. Additionally, you will have the chance to contribute to the company's culture of excellence and innovation, working closely with a dedicated team to improve patient care and outcomes. Must be willing to train for inpatient experience.

Key Responsibilities:

  • Develop, implement, and maintain social services policies, procedures, and programs, including discharge planning services.
  • Oversee social services case management activities and ensure staff competency through development and in-services.
  • Recommend and implement new programs to improve services for specialized patient populations and reduce hospital and office visit utilization.
  • Manage departmental budget and identify opportunities for cost reduction and service improvement.
  • Identify and utilize community resources to support our members and collaborate with community organizations to enhance services.
  • Monitor and ensure the quality assurance of social services programs and staff, resolving any issues that may interfere with service delivery.
  • Collaborate with medical center departments, community agencies, and health plan members on discharge planning to ensure integrated service provision.
  • Ensure compliance with federal, state, and local requirements.

Qualifications:

  • Minimum two (2) years of leadership or supervisory experience in medical social work.
  • Licensed Clinical Social Worker (California) required at hire.
  • Driver's License (California).
  • Basic Life Support certification.
  • National Provider Identifier required at hire.
  • Strong knowledge of fiscal management theories and principles.
  • Understanding of different levels of health care and human development and psychodynamic theories.
  • Ability to work in a Labor/Management Partnership environment.
  • Minimum five (5) years of supervisory experience in medical social work preferred.

Compensation and Benefits:
