Overview:
Licensed Master Social Worker
Full Time
M-F -Days
Honolulu, HI

The Licensed Master Social Worker provides social services to patients and their families in a timely and efficient manner in order to promote maximum social functioning and psychological adjustment to dialysis and rehabilitation of the patient.

Responsibilities:
-Assist with clinical and operational processes to improve patient health and minimize hospitalizations and missed treatments.
-Coordinate transient arrangements.
-Demonstrate effective use of supplies.
-Perform duties as assigned to meet the patient care or operational needs of the clinic.
-Identify and counsel psychosocial issues.
-Provide patient and family education.
-Assist with program's target goals for patient outcomes in accordance with quality patient care and Company goals.

Requirements:
-Must have Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
-Current licensure in HI-Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW)
-Prefers previous experience in providing social services to dialysis patients preferred.

Things you should know about the job:
-Will take new grads as long as they have completed their MSW and have the LSW HI license.
-Schedule can be flexible, but usually 8:30 am-5 pm
