A well-respected regional healthcare system in the Florida Panhandle is seeking a skilled Social Worker to join their multidisciplinary team at a top-performing, 267-bed hospital. This facility is renowned for advanced care programs including a Primary Stroke Center, Cancer Care Center, open-heart surgery, and specialized emergency services.

You’ll be part of a compassionate team committed to delivering comprehensive care across inpatient units and the emergency department, serving a diverse patient population with complex medical and psychosocial needs.

Position Overview
This full-time role primarily covers daytime shifts with rotating weekends, typically 8:00 AM – 4:30 PM, with occasional variations. You will provide non-clinical biopsychosocial assessments, crisis intervention, discharge planning, and support services across various inpatient units including ICU, Trauma, Oncology, NICU, Transplant, and the Emergency Department. This position focuses heavily on discharge planning experience and coordination of post-acute care resources.

Work Schedule
Day shifts with rotating weekends, typically 8:00 AM – 4:30 PM; some shifts may vary slightly (9:00 AM – 5:30 PM or 9:30 AM – 6:00 PM).

Core Responsibilities

  • Conduct thorough biopsychosocial assessments for inpatient and emergency department patients

  • Provide crisis intervention, emotional support, and family counseling during complex medical situations

  • Collaborate closely with physicians, nursing staff, case managers, insurance providers, and community resources

  • Develop and implement discharge plans addressing barriers including financial, emotional, and psychosocial needs

  • Coordinate post-acute care placement and assist with patient/family decision-making

  • Maintain knowledge of Medicare, Medicaid, and managed care regulations to ensure compliance and fiscal responsibility

  • Participate in interdisciplinary team meetings and provide advocacy for patient-centered care
