We are a Federally Qualified Health Center (FQHC) dedicated to delivering comprehensive and compassionate care to our community in Washington County, Pennsylvania. We are currently seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to join our integrated care team. Responsibilities:
- Provide outpatient behavioral health services to a diverse patient population.
- Conduct assessments, develop treatment plans, and deliver individual, group, and family therapy.
- Collaborate with primary care and specialty providers as part of a fully integrated healthcare team.
- Maintain accurate and timely documentation in accordance with federal and state guidelines.
- Participate in case reviews, staff meetings, and quality improvement initiatives.
Qualifications:
- Active Pennsylvania licensure as an LCSW or LPC required.
- Experience in community-based or outpatient behavioral health preferred.
- Strong communication and clinical skills.
- Commitment to serving underserved populations.
