Pay: $100,000 - $120,000/Year
Director of Case Management Opportunity:
- Full-time permanent position
- Director of Case Management manage the daily operations and HR functions of the Case Management department
- Directs the interdisciplinary care, discharge planning, and patient outcomes while championing patient advocacy
- Guides and trains staff, builds relationships with healthcare and insurance networks, and fosters a positive, celebratory work culture
- Must be a licensed clinician (ex: RN, SW, OT, PT, ST, Rehabilitation counseling, etc.)
- Current CCM or ACM certification or eligibility to obtain within first year of employment
- Must possess a bachelor's degree; graduate degree is preferred
- Three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience
