Job Description:

We are seeking a compassionate and professional Social Services Director to join our skilled nursing facility team. The ideal candidate will oversee the provision of social services to our residents, ensuring their emotional and social needs are met in a confidential and professional manner. This role demands a high level of discretion, empathy, and a strong commitment to the dignity and privacy of our residents.

Key Responsibilities:

  • Develop, implement, and evaluate the facility's social services programs based on resident needs, facility objectives, and regulatory standards.
  • Conduct comprehensive assessments of resident social and emotional needs, developing individualized care plans in collaboration with a multidisciplinary team.
  • Provide counseling and support to residents and their families, navigating complex health care decisions and transitions.
  • Advocate for residents' rights and well-being, acting as a liaison between residents, families, and facility staff.
  • Coordinate with external agencies to facilitate services such as home health care, medical equipment, and community resources.
  • Maintain accurate and confidential resident records, ensuring compliance with federal, state, and local regulations.
  • Lead and train social services staff, promoting a culture of compassion, professionalism, and respect.
  • Participate in facility quality assurance programs, contributing to the improvement of overall care and services.

Qualifications:

  • Bachelor's or Master's degree in Social Work or a related field from an accredited institution.
  • Valid state licensure as a Social Worker, if applicable.
  • Minimum of 3 years of experience in social work, preferably in a health care setting, with at least 1 year in a supervisory or management role.
  • Strong understanding of the social, psychological, and recreational needs of the elderly.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse individuals and groups.
  • Proficient in maintaining confidential information and understanding of HIPAA regulations.
  • Demonstrated leadership skills and the ability to motivate and direct a team.
  • Ability to navigate complex situations with sensitivity, tact, and professionalism.
