Assist in the discharge planning process for patients and work closely with the Chronic Disease Specialist.
Responsibilities: Assessment, Planning, Collaboration, Social Work Services including interventions and documenting Medical Records.
-Must have current License as Social Work but Prefer NY Licensure as MSW -Must have 3 years experience as social worker working in a healthcare environment
