Job Description
PT needed for SNF position in Hartford, AL with an 5/28 start date
• Candidate presented must have active State Licensure or proof license is in process. If license is active please include proof that it is in good standing with the board.
• Candidate presented must have Completed Resume or Agency Profile including detailed Work History, 2 Completed References, and Completed Skills Checklist.
• Please acknowledge if you candidate is proficient in Casamba and/or Rehab Optima when presented
• Productivity expectation for contractors on assignment with Diversicare is 90% or above
• Please review the Diversicare Uniform policy under the Facility Profile page in Medefis. It is Scrubs with Royal Blue top and Black Bottoms. This dress code is mandatory. If you are unable to find this document please message us.
