(Archived) Physical Therapist

We are working together to provide quality care for our patients through regular visits.
The ideal candidate is a skilled professional that is passionate about delivering quality care and making a real difference.

The candidate will be assigned to either the Western or Eastern part of Phoenix depending on preference and/or location. We know that our providers are looking for flexibility during these times and are happy to discuss personal preferences in terms of scheduling, etc. in an interview.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from doctors or surgeons
• Diagnose movement dysfunction by listening to patients and observing them as they move about
• Coach patients and teach them therapeutic exercises and stretches to improve their condition
• Use hands-on therapy such as massage to ease patients’ pain and provide stimulation to promote healing
• Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly
• Advise patients and their families on in-home treatment options and what to expect from the recovery process
• Assess each patient and develop an appropriate treatment plan with measurable goals and time frames
• Prepare and submit clinical and progress summaries based on identified and attained goals
• Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient

Qualifications

• Bachelor's, Master's or Doctor's degree of an accredited physical therapy curriculum
• Certification in Physical Therapy and valid state license to practice
