Physical Therapist - Skilled - (PT - Skilled)
StartDate: ASAP Pay Rate: $1400.00 - 1800.00

Not accepting new grads. SNF experience required. 13-week contract, but 26 weeks would be better. Rehab Optima Experience Preferred. State License Verification. Resume. References (3). Flu Shot (During Flu Season Only). Federal Criminal (within 30 days of start). Elder Justice Act, read (signed upon confirmation). HHS/OIG Search. GSA Search. Two Forms of ID. True 2-Step PPD or Quantiferon (2nd PPD must have been placed within 7-14 days after the first PPD was read). Physical (within 6 months of start, must state "able to work with no restrictions and free from communicable diseases"). Idaho State Background [Register to View] Qualifications
