Job Description
Overview
The Physical Therapist plans and administers medically prescribed or referred physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury, surgery, or loss of body part. Must document patient care, educate and inform patient and referring provider on outlined plan of care.
Qualifications
Education:
- Graduate of an accredited program in Physical Therapy
- Bachelor's or Master's degree preferred
Experience:
Licenses/Certifications:
- Current and unrestricted Physical Therapy license in the state
- Current CPR Certification
Responsibilities
- Develops and implements a rehabilitation program consistent with the physician's plan of treatment for patients
- Confers with physician and clinical team members to obtain additional patient information and assist in developing, implementing and revising the therapy treatment program and plan of treatment.
- Instructs, motivates and assists patients in non-manual exercises.
- Administers manual therapeutic exercise.
- Maintains daily progress notes, correspondence with provider and Wellpath documentation procedures.
