POSITION SUMMARY: Directs, supervises, evaluates, and provides physical therapy services to the patient in their home as established in the Plan of Care. Adheres to established physical therapy principles and practices in the planning and provision of services. Understands the stages of growth and development from pediatrics to geriatrics, and adjusts care and treatment accordingly. Maintains accurate records of the care provided. Ensures the safe delivery of home health care services. The incumbent may perform other duties that are not mentioned above, and specific functions may change from time to time. Instructs and supervises Home Health Aides and Licensed Physical Therapy Assistants in the provision of specific therapies. Collaborates with the RN Care Manager in the coordination of multi-disciplinary services.

QUALIFICATIONS: Graduate of Physical Therapy Program culminating in a minimum of a Bachelor of Science Degree. Must be licensed or eligible for licensure in the State of West Virginia. Two (2) years of clinical experience in physical therapy preferred. Previous home health experience appreciated, but not required. Current CPR certification at the BCLS level according to American Heart Association standards required. Requires a valid driver's license in good standing and a dependable vehicle with automobile insurance that meets the requirements of West Virginia law.
