A leading home health organization in Portland, Oregon is seeking a Home Health Physical Therapist to join its growing team!
Pay: $65+/hourly
Home Health Physical Therapist Opportunity:
- Full-time, permanent, direct-hire position
- Monday-Friday, 8:00 AM - 5:00 PM schedule
- Guaranteed weekly pay
- Mileage reimbursement provided
- Relocation assistance available

- Evaluate and assess patients' physical conditions and develop individualized treatment plans
- Implement therapeutic exercises and techniques to improve mobility, strength, and coordination
- Educate patients and caregivers on exercises, safety precautions, and self-care strategies
- Collaborate with an interdisciplinary healthcare team to provide patient-centered care
- Maintain accurate and timely documentation of patient progress and treatment plans
- Active Oregon Physical Therapist license required
- Master's or Doctorate Degree in Physical Therapy required
- Minimum of one year of Physical Therapy experience required
- Home health experience strongly preferred
- Current BLS/CPR certification required
- Valid driver's license and reliable, insured vehicle required
- Serves the Gresham and greater Portland area
