Position Overview

We are seeking a motivated and compassionate Outpatient Physical Therapist to join an independently owned physical therapy practice in Fort Wayne, Indiana.

Key Responsibilities

• Evaluate patients and develop individualized treatment plans based on their specific needs and goals.

• Provide one-on-one physical therapy treatments using evidence-based techniques.

• Monitor patient progress and modify treatment plans as appropriate.

• Educate patients and families on exercises, home programs, injury prevention, and recovery.

• Maintain accurate and timely clinical documentation.

• Collaborate with other members of the therapy and healthcare team.

• Build strong relationships with patients while providing compassionate, high-quality care.

• Participate in ongoing professional development and mentorship opportunities.

Qualifications

• PT or DPT from an accredited program

• Active Indiana Physical Therapist license

• Strong communication and interpersonal skills

• Team-oriented personality

• Commitment to patient-centered, high-quality care

• Ability to work independently while contributing to a collaborative team
