An established outpatient rehabilitation setting is seeking a Physical Therapist to evaluate, plan, and deliver high-quality physical therapy services. This role focuses on patient-centered care while adhering to professional standards, regulatory requirements, and best practices in physical therapy.

Key Responsibilities:

• Evaluate patients based on provider referrals and establish individualized plans of care

• Deliver safe, effective physical therapy treatments with timely and accurate documentation

• Communicate patient progress and clinical status with patients, families, and care teams

• Document evaluations, treatments, progress notes, and discharges

• Maintain a clean, safe treatment environment and ensure equipment readiness

• Collaborate with physicians and notify them of changes in patient condition

• Supervise and support PTAs, rehab technicians, and students as appropriate

• Provide patient and family education related to treatment plans and goals

• Participate in department meetings, case conferences, and ongoing professional development

• Perform other duties as assigned

Qualifications:

• Bachelor’s Degree in Physical Therapy

• Active Physical Therapist license

• Current AHA BLS certification

• Ability to pass aquatic competency testing within 90 days if aquatic therapy is required
