Job Description
A beautiful facility near Baker City, OR is seeking a full-time Physical Therapist – Home Health to join their team!
- Pay: $40.00 - $57.00 hourly
- $10,000 retention bonus + relocation assistance on a case-by-case basis!
Physical Therapist – Home Health Opportunity:
- Full-time, permanent, direct-hire
- Schedule: Day shift, 5x8 or 4x10
- In-home patient visits are required for the Physical Therapist – Home Health.
Physical Therapist – Home Health Responsibilities:
- Evaluate mobility, strength, balance, and functional abilities to determine care needs.
- Develop and implement individualized treatment plans in collaboration with the patient, family, and care team.
- Provide skilled, goal-oriented physical therapy with ongoing assessment and plan adjustments.
- Document care per regulatory guidelines and coordinate with interdisciplinary teams.
Physical Therapist – Home Health Qualifications:
- Bachelor's degree from an accredited Physical Therapy program
- Valid PT license, or eligibility to obtain
- BLS Certification, or eligibility to obtain
