PHYSICAL THERAPIST JOB REQUIREMENTS:

  1. Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program
  2. Successful completion of National Certification Examination
  3. 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
  4. Home care experience
  5. Valid PT license in the state of practice
  6. BLS
  7. CPR Certification
  8. Knowledge of electrical safety and infection control guidelines.
  9. Lymphedema experience required
  10. Must have reliable transportation and insurance
DUTIES:
A registered professional who provides physical therapy to patients of all age groups. Plans, implements and evaluates patient care plans to restore or maintain patient well being. Plans, organizes and conducts physical therapy treatment program based on the medical referral and their evaluation. Collaborates with interdisciplinary team. Essential Functions: 1. Performs clinical assessment to identify patient needs and appropriateness of patient. 2. Document, prepare/review and complete a Plan of Care (POC), progress notes, phone orders and other clinical record documentation on an ongoing basis. Revise the POC based on evaluation, ongoing assessment data. Works in consultation with the physician and as part of an interdisciplinary team. 3. Plan and provide appropriate treatment for neuromuscular, neurological and orthopedic disorders based on medical referral and therapy assessments. 4. Assist/instruct patient/family in transfer techniques appropriate to patient abilities. Instruct on use of equipment such as walkers, wheelchair, canes, braces, prosthetic devises and appliances in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs. Orders appropriate rehabilitative equipment to meet patients’ needs and adapts equipment, appliances to meet home limitations/requirements. 5. Provides appropriate patient/family education regarding therapeutic procedures based on the patient’s age, condition, culture and learning readiness. 6. Performs and oversees other team members, when applicable, in the performance of patient care that is consistent with patient’s needs and desires. 7. Applies concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease. 8. Accepts clinical assignments that are consistent with education and competence to care for patients. 9. Performs other related duties as assigned or requested. Knowledge/Skills/Abilities: 1. Uses effective interpersonal relations and communication skills. 2. Knowledge of durable medical equipment. 3. Meets mandatory continuing education requirements. 4. Demonstrates commitment, professional growth and competency. 5. Ability to work flexible schedule and travel locally. 6. Ability to do heavy lifting, bending, pulling, pushing and standing. Prolonged standing and walking required. Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient/family individualized psycho social needs. 7. Requires hand-eye coordination and manual dexterity. 8. Promotes company philosophy and administrative policies. 9. Must read, write and speak fluent English.
