PHYSICAL THERAPIST JOB REQUIREMENTS:

  1. Masters of Science or Doctorate in Physical Therapy from an accredited program
  2. Successful completion of National Certification Examination
  3. 1 year of verifiable, supervised professional experience within the last 3 years
  4. Valid PT license in state of practice

JOB SUMMARY:

Under the supervision of the Clinical Supervisor, the Team Nurse/Therapist is responsible for the delivery of appropriate, adequate, effective, efficient direct patient care services assigned by the Primary Care Clinician and approved by the Clinical Supervisor. Responsibilities include implementation of the physician plan of care, obtaining physician orders, supervision of home health aides, and coordination of care within an interdisciplinary team and participating in effective utilization management and control of patient supplies. Consistently provides care based on the age-related needs of the unique population with consideration of their development/psychosocial needs, environmental safety, educational needs, communication style, and level of understanding. The Team Nurse/Therapist is accountable to the Primary Care Clinician for ensuring that the plan of care is implemented following professional standards of care and agency specific protocols. MINIMUM POSITION REQUIREMENTS: Knowledge: Exhibits excellent oral and written communication skills: with legible handwriting for documentation of patient records in a clear and concise manner. Basic understanding of home care services, clinical standards of care and practice, Title XXII, Medicare Conditions of Participation, JCAHO home health accreditation requirements, QI outcomes/benchmarking, infection control; good understanding of reimbursement patterns for Medicare and other payors; good working understanding of budget and other financial reports. Special Skills/Equipment: Self-starter with a high degree of initiative, motivation, flexibility, energy and creativity. Ability to form harmonious working relationships with internal and external customers. Demonstrated leadership and negotiation skills and the ability to manage resources for patient care in a cost effective manner. Excellent collaborative and problem solving skills with customers and management. Ability to exercise independent sound judgment in planning and providing patient care. Demonstrated ability in physical, psychosocial, and environmental assessment skills and in implementing plan of treatment. Excellent organizational skills. Must be able to meet company productivity standards as indicated by organization and manager. PRINCIPAL ACCOUNTABILITIES: Primary Job Responsibilities: • utilizes effective physical and psychosocial assessment skills, performs and documents multi-system assessments including OASIS • identifies and documents realistic, measurable and appropriate patient goals in collaboration with patient/caregiver • seeks appropriate consultation for insufficient progress towards patient identified goals • provide the patient with effective patient/caregiver instruction according to patient/caregiver learning needs • works collaboratively with other clinical and clerical team members to meet patient plan of care and/or agency mission and objectives • notifies PCC or DPCS in a timely manner of emergency, unexpected or significant changes in the patient medical condition Quality Management (includes documentation and regulatory standards) • completes all admission paperwork in a timely manner including consents, billing and clinical forms per agency policy and procedure • appropriately implements and updates the plan of care in a timely manner in accordance with accepted professional standards of practice in collaboration with the PCC • monitors and ensures positive patient outcomes for each plan of care intervention • actively involves patient in the development of the plan of care and adequately documents same in the patient clinical record • ensures patient notification of changes in the visit schedule
