Job Description

Optometrist

Location: Naval Base Portsmouth, VA

Work Schedule: Services shall be provided Monday through Friday, between the hours of 0700 and 1900. Shifts will normally be scheduled for an eight and half (8.5) to nine (9) hour period, to include an uncompensated thirty (30) minute to sixty (60) minute lunch break, respectively. In no instance will the HCW be required to provide services in excess of eighty (80) hours per two-week period. Services shall not be required on federal holidays when the clinic is open.

Qualifications:

5.2.1.4.2. Possess a minimum of one (1) year full-time experience fitting specialty lenses for keratoconus within the preceding eighteen (18) months.

5.2.1.4.3. Possess a certification to use Therapeutic Pharmaceutical Agents.

5.2.1.5. In Lieu of the qualifications listed in the contract, the following additional or supplemental qualifications are required for HCWs on this TO:

5.5.3.2.6.1. Perform a full range of optometry services. Routine workload is scheduled by the treatment facility through the appointment system and subject to the approval of the department head. Secondary workload is a result of consultation requests submitted to the facility by other staff health care providers. In addition to clinical responsibilities, the HCW will be assigned administrative tasks commensurate with the position.

5.5.3.2.6.2. Be evaluated annually on their performance and adherence to the requirements of the task order. However, the government reserves the right to evaluate the HCW semi-annually. Additional (i.e. special) evaluations may be performed to correct clinical or other performance deficiencies identified by the government. The totality of scheduled performance evaluations, any special evaluations, memoranda of counseling sessions, and any other documentation generated by the government will constitute the complete evaluation.

5.5.3.2.6.3. Perform administrative functions such as serving on boards and committees and attending or providing continuing education.

5.5.3.2.6.4. Provide optometric care in accordance with the community standard of care and American Optometric Association (AOA) clinical practice guidelines. This shall include performance of all necessary tests and patient work-up to adequately diagnose patients. Examples include:

Performing basic visual acuity tests

Measuring and recording distance and near vision, color testing and pinhole vision

Lensometry

Keratometry

Refraction (manual, automated and retinoscopy)

Ocular motility tests

Binocular coordination testing (vergences, phorias, accommodation)

Visual field testing - automated and manual

External examination

Slit lamp examination

Applanation tonometry

Funduscopic Examination (Indirect and Direct)

Corneal topography

Pachymetry

5.5.3.2.6.5. Accurately transcribe into the patient medical record, past ocular history, adverse drug reactions, allergies, etc. according to established techniques and procedures. Identify patient’s chief complaint and provide a written assessment of findings. Diagnosis and recommendations should be appropriate for the complaint and clinical findings. Appropriate follow-up should be indicated, patient education must be documented, and provider stamp and signature must be present.

5.5.3.2.6.6. Diagnostic Pharmaceutical Use: Provide accurate documentation of specific medication used, dosage, and time given. Document any adverse reaction.

5.5.3.2.6.7. Provide Dilation on patients following the below outline:

Asymptomatic low-risk patient: Every 3-5 years as needed under age 50

Every 1-3 years age 50 and over

Asymptomatic high-risk patient: Every 3-12 months as needed

Symptomatic patient: Promptly

Specific indications:

- Previous diagnosis of retinal detachment, retinal holes, or lattice degeneration

- History of significant trauma to the eye or orbit

- Myopia in excess of -5.00 Diopters

- Lumps behind the iris

- Aphakia or pseudophakia (except with iris-fixed IOL's)

- History of drug use that would cause adverse ocular effects:

  • Cataractogenic substances (e.g. corticosteroids)
  • Retinotoxic substances (e.g. hydroxychloroquine)

- History of significant atherosclerosis with cardiac/carotid manifestation or CVA

- History of diabetes mellitus

- Complaints of flashes or floaters. (high risk)

- Presence of a media opacity preventing a good view of the retina

5.5.3.2.6.8 Detect neuromuscular impairment or anomalies of binocular function.

5.5.3.2.6.9. Provide pre-op and post-op care of corneal refractive surgery patients to include LASIK, PRK and implantable contact lenses (ICL).

5.5.3.2.6.10 Prescribe and dispense medications in accordance with privileges and as delineated by the Pharmacy and Therapeutics Committee.
