Small, Rural Community Hospital in Arkansas is in search for an experienced RN Director of Case Management. Interested candidates should have previous Case Management level experience in the acute care setting. BSN preferred.

Responsibilities of the Director of Case Management:

• Provides case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment.
• Coordinate the integration of social services/case management functions into the patient care, discharge, and home planning processes with other hospital departments, external service organizations, agencies, and healthcare facilities
• Communicate with physicians at regular intervals throughout hospitalization and develops an effective care plan to maintain appropriate cost, case and desired patient outcomes
• Supervises the duties and schedule of the Utilization Review Nurse

Requirements of the Director of Case Management

• Previous RN Case Management acute care setting experience
• Previous RN Leadership experience
• BSN or related degree preferred
