TITLE: Director of Care Management(Case Management)
LOCATION: Alberton, MT

The area boast a population around 75k and the hospital several service lines that a larger acute hospital would offer like a cancer center, wound care clinic, ambulatory, inpatient rehab, L&D and Pediatric ICU.
The hospital serves the residents of the community and you can be part of the team that focus' is on patient safety and delivering the highest quality care.
SUMMARY:
The Director of Care Management is accountable for the implementation, coordination and management of the Department.
Responsible for the programmatic development, planning, organization and management of the functions that focus on care facilitation and movement of patients through the hospital continuum.
This includes, but is not limited to: Case Management, Social Work, Utilization Management and post-acute services coordination.
Develops the Hospital Case Management program to enhance the distinction in staff roles related to progression of care.
QUALIFICATIONS:
Must have a RN and a BSN.
Master's degree is preferred.
3-5 years case management in a hospital setting. 2-3 years in a leadership position.
ADDITIONAL INFO:
Facility is around 100 beds and census averages 75.
The position reports to the CNO and CFO and oversees 7 FTE's. The leadership team is wonderful and what makes this facility a great place to work.
Looking for someone that can be a leader with a "we" attitude as they would be a team. Person needs to be a resource for the group.
Experience with Bedside Case Management, Utilization Review, Discharge Planning and Denials as this is all part of what this person is overseeing.
The range for the position is between 110K-120K and they offer full benefits including relo and sign-on.
