Salary Range: $62,700 - $111,600 (based on experience)
Social Worker - Case Manager Opportunity:
- Full-time Permanent hire
- Schedule: Monday-Friday 8a-5p
- $5,000 Signing Bonus
- Social Worker - Case Manager coordinates and oversees patient care plans by collaborating with the healthcare team to ensure efficient, high-quality care and smooth transitions throughout the hospital stay.
- Social Worker - Case Manager must have an unrestricted RN license
- Previous experience in an inpatient rehab/hospital setting preferred
- Previous case management experience required
- Convenient Location: Ideally situated between Boston and Worcester, offering easy access to both cities via the Mass Pike (I-90) and commuter rail service
