Health Promotion Coordinator Job Overview:

  • Strengthens human performance and total force readiness by providing healthy lifestyle coaching, and health awareness, education, intervention programs to reduce healthcare demands and costs.
  • Intervention focuses on reducing costs related to metabolic disease states, thereby promoting a community environment that facilitates health instead of the need for constant healthcare.
  • Interventions targeted at Military Health System (MHS) beneficiaries, including active duty, family members, and retirees.
  • Responsible for planning, coordinating, implementing, and evaluating all Healthcare to Health (H2H) (i.e. Group Lifestyle Balance) Health Promotion (HP) prevention programs and services IAW Air Force instructions, guidelines, and population needs
  • Administers assessments, coaching/counseling, and various other health and nutrition programs/activities that meet the needs of the installation community at large.
  • Coordinates with H2H & HP staff to deliver programs in a variety of community settings to include the MTF, community facilities, and individual unit worksites
  • Implements and executes H2H/HP program requirements
  • Facilitating classes and activities. Classes and activities, as well as, participating in health fairs/community outreach events may occasionally occur outside the normal duty day.
  • Teaches individuals and groups on a variety of health-related topics employing age-specific and behavioral learning theory
  • Participates in teleconferences, updates, working groups, review panels, symposiums, etc. related to H2H/HP programming or operations
  • Maintains data quality management and reporting requirements in program execution
  • Accesses available population data from appropriate databases and collaborates with population health planners to effectively assess population needs and target H2H/HP programming for individuals and groups.
  • Collects and submits H2H/HP program data as requested by the Government.
  • Applies appropriate social marketing principles to implement health communication campaigns to promote the H2H/HP program.
  • Influences Tricare beneficiaries' knowledge, attitudes, and social norms surrounding health-related behaviors.
  • Utilizes traditional media (print) and social media for health communication campaigns and to promote the HP/H2H program.
  • Maintains positive relationships with both internal and external MHS customers.
  • Provides and promotes excellent customer service.
  • Establishes, builds, and maintains relationships with a variety of installation organizations to execute the H2H/HP program goals and objectives.
  • Communicates and coordinates H2H/HP programs among local leadership and target customers/beneficiaries.
  • Delivers H2H/HP briefings, educational sessions, and seminars/workshops as required/requested
  • Provides expertise to develop, deliver and evaluate training programs for professional staff and colleagues on H2H/HP related topics.
  • Actively participates as a member of the installation IDS team and Population Health Working Group (PHWG) as appropriate.
  • Collaborates with MTF health professionals and organizational leaders to identify and employ resources as appropriate to execute H2H/HPP.
  • Operates the most current computer software and multi-media to develop and execute training and educational programs.

Health Promotion Coordinator Job Qualifications:

  • Basic Life Support (BLS)
  • Bachelor's Degree in Health Education, Health Promotions, Community Health Education, or Public Health.
  • Graduate from an accredited college or university
  • Eligible for Certified Health Education Specialist (CHES) from the National Commission for Health Education Credentialing (NCHEC)

  • Minimum of three years' experience in health promotion planning and programming, including obesity & weight management and lifestyle counseling, including knowledge of the Diabetes Prevention Program Outcomes studies & the National Diabetes Prevention Program.

  • Microsoft Office competency
