A internal medicine clinic in central Oregon is looking for a Behavioral Health Consultant (LPC, LMFT, or LCSW) to work in collaboration with primary care providers to engage in direct patient care, coordination of mental health services, psycho-education, and group visits.
A clinic in central Oregon is looking for a Behavioral Health Consultant. This position would require working as part of a multi-disciplinary team to support the physical, psychosocial, and behavioral needs of a diverse and under-served patient population within a primary care setting. This person would engage in patient care, coordinate mental health services as needed, provide coaching and psycho-education, as well as group visits.
Responsibilities
- Works with other members of a primary care team to treat and manage patients with chronic behavioral, physical, and social conditions.
- Assists in the planning, strategy, and development of patient care plans to prevent further psychological and physical deterioration through referrals and consistent consultation.
- Assists in clinical assessments.
- Educated patients, families, and clinical staff regarding techniques related to self-care.
- Uses current evidence-based guideline to create diagnostic and treatment plans.
Skills and Knowledge
- Knowledge of behavioral medicine and treatments for medical, mental health, and substance abuse conditions.
- Strategizes and provides psychologicl assessment and diagnosis with brief psychotherapeutic interventions as needed.
- Advises and consults the primary care providers and team members.
- Skilled in support group organization, fascilitating, as well as educating.
- Be comfortable in the role as consultant and resident BH specialist.
Education/Experience
- Masters or better
- 2+ years of delivering behavioral health services to medically underserved populations and cultural competency in a medical setting
- LCSW
- Licensed Professional Counselor
