Physical Therapist - Home Health - (PT - HH)
Acute Hospital-Based Home Health department is seeking a self starting, highly motivated Physical Therapist to join the team. Therapist will treat an adult and geriatric population seeing approximately 5-7 patients per day. Candidates must be willing to support a friendly, positive and professional environment.

A physical therapist (PT) is responsible for diagnosing and treating patients of all ages who suffer from physical conditions that limit the ability to move and/or perform daily functional activities. A PT has a number of job functions, including assessing the patient and creating an individualized treatment plan to work on the areas that could use improvement. In assisting with a patient’s physical therapy, a PT also incorporates preventative measures against the loss of motor skills and promotes overall healthier lifestyles. Required Qualifications
Home Health experience required. WA License required.
Preferred Qualifications
Prefer at least 6 months Home Health experience with OASIS C.
