Job Description
Job Title: Social Worker
Location: Beloit, WI
Overview:
Our client's team is growing and we need Masters of Social work for Day Shift. This position offers a competitive wage, with generous benefits, all while working for a Health System with over 50 years of experience that is known for our friendly atmosphere and focus on patients.
What you will be doing:
- In this position, the Social Worker will be responsible for Hospice Patient assessment and implementation of the plan of care.
- The Social Worker will work to provide direct Case Work Services.
What you will be doing:
- Master's Degree in Social Work required
- WI/IL Social Work License is required
- State of WI letter approval of Psychotherapy Services in Certified Out-Patient Mental Health Clinic
- Evaluation and treatment for pediatric, adolescent, adult and geriatric population
- Valid Driver's License
2 years or more of experience is preferred.
